Was the amount of the loss over $10,000 dollars?	Yes No
Was your personal identity,credit or debit card used for fraudulent activity?	Yes No

Upon completion of this process you will:

See the words: "Your report has been submitted" showing that your report is complete.
Receive a confirmation email containing a temporary report number.
